My passport was lost or stolen. How do I get a new one?
Please apply for a new passport immediately. You may report your lost or stolen passport online at travel.state.gov or when you apply for a new passport must submit the Statement Regarding Lost or Stolen Passport, Form DS-64. Lost or Stolen Passport Form DS-64

Where do I get a passport application?
Applications and information are available on the 2nd floor lobby of the City Clerk's office, Moreno Valley City Hall, 14177 Frederick St, Moreno Valley.

How long does it take to get a passport?
Standard processing is 10-12 weeks.
Expedited processing is 4-6 weeks. -
How do I renew my passport?
For those eligible to renew, Passport renewals are processed by mail.
You'll need:
- Completed Form DS-82
- Most recently issued passport
- A recent passport photo
- A check for the passport fee

How do I replace a lost/stolen passport?
Please apply for a new passport immediately.
You may report your lost or stolen passport online at travel.state.gov. You will need to use Lost or Stolen Passport Form DS-64

How long is a passport valid?
If you were 16 or older when the passport was issued, then your passport is valid for 10 years and you are eligible to renew your passport within 15 years from the date of issue.
If you were 15 or younger when the passport was issued, then your passport is valid for 5 years and you are not eligible to renew your passport but must reapply as a new applicant.

How do I get a certified copy of my birth certificate?
Contact the Vital Statistics office in the state where you were born.

How do I get a pasport for my child?
Children 15 yrs. old and under:
Both parents or legal guardians, must be present with the minor. Both parents or legal guardians must show valid I.D. and proof of relationship (e.g. birth certificate, adoption decree, or court documentation, etc.)
Children 16 and 17 yrs. old:
Your child must appear in person with one parent's consent and present their/parent's valid I.D. -
Do I have to provide my Social Security Number?
Yes. Section 6039E of the Internal Revenue Code (26 U.S.C.6039E) requires you to provide your Social Security Number (SSN), if you have one, when you apply for a U.S. passport or renewal of U.S. passport.

How can I find the status of my passport application?
Status updates for passport applications are available 7-10 days after you apply or renew.

I was recently married/divorced. How do I change my name on my passport?
You will need to complete DS-5504 form to make any data corrections within one year of the issuance date of your current valid passport and submit along with the following:
- The passport to be replaced
- Certified documentation of your name change (e.g., marriage certificate, divorce decree with your new name)
- A recent passport photo
If the issuance date exceeds one year, the passport must be renewed using form DS-82, if eligible, or you may reapply for a new passport using form DS-11.
